The purpose of this role is to ensure the delivery of high-quality, person-centred support to NDIS participants and ensure that the services being rendered by Incluso Services team members meets our quality and care standards. This role involves leading a team of Disability Support Workers, coordinating the service delivery, rostering your team, maintaining required compliance and fostering a positive and inclusive environment that promotes participant independence and community participation for the participant/s that reside in the assigned SIL home. 


RequirementsWhat You'll Do
  • Oversee the rostering and delivery of support services across the assigned SIL home.
  • Ensure all participant supports are person-centred, goal-oriented and aligned with their NDIS plans and individual needs.
  • Manage forward rostering and respond to unexpected changes in staffing and shifts to ensure no support is missed.
  • Monitor participant progress, wellbeing and changing support requirements.
  • Regularly review case notes to ensure accurate record keeping, quality and compliance.
  • Maintain high standards of health, safety, care and cleanliness within the home.
  • Attend case meetings and provide regular updates to guardians, support coordinators, families and external stakeholders.
  • Recruit, train, supervise and support a multidisciplinary team of support workers.
  • Manage team performance, staffing levels, rosters and professional development requirements.
  • Foster a positive, inclusive, collaborative and learning-focused team culture.
  • Support communication between participants, families, staff and external stakeholders.
  • Build and maintain positive relationships with participants and their support networks.
  • Ensure all services comply with NDIS guidelines, organisational policies and quality and safeguarding requirements.
  • Monitor risk management, incident reporting, feedback and complaints, workforce capability and other compliance requirements.
  • Conduct regular checks of the SIL home to maintain high standards of care, health, safety and presentation.
  • Monitor medication administration records and ensure compliance with relevant policies.
  • Record and monitor restrictive practices, including interim arrangements.
  • Maintain accurate participant health profiles, hospital records and chronic condition documentation.
  • Ensure participant documentation is complete and up to date in Visual Care.
  • Prepare reports on team performance, service delivery and participant outcomes.
  • Support audits, inspections and NDIS Quality and Safeguards Commission assessments as required.
  • Provide occasional support with urgent enquiries outside normal business hours.
What You'll Bring
  • Certificate IV in Disability, Community Services or a related field.
  • Strong understanding of disability support services, NDIS plans and person-centred care.
  • Experience coordinating support services and staff rostering.
  • Previous experience leading, supervising or supporting a team.
  • Strong understanding of NDIS compliance, quality and safeguarding requirements.
  • Experience using NDIS or healthcare-specific CRM or case management systems.
  • Excellent communication and interpersonal skills.
  • Strong ability to build positive relationships with participants, families, staff and external stakeholders.
  • Excellent organisational skills, with strong attention to detail and accurate record-keeping capability.
  • Ability to manage competing priorities and respond confidently to changing circumstances.
  • Strong problem-solving skills and a proactive, solutions-focused approach.
  • Excellent communication and presentation skills, with the ability to explain complex information clearly.
  • A friendly, positive and can-do attitude.
  • Genuine commitment to person-centred support and positive participant outcomes.
  • Ability to work collaboratively as part of a fast-paced team environment.
  • Diploma or higher qualification in a relevant discipline is desirable.
