The role involves supporting dementia patients transitioning from hospital to home, providing assessments, coaching caregivers, and developing tailored strategies to improve patient care and quality of life.

Provide person-centred residential aged care under the Clinical Care Manager: administer medicines, support residents’ health, wellbeing and dignity, follow Aged Care Quality Standards, maintain documentation, build relationships with residents and families, and work collaboratively with the care team.
